EDIT: Sheesh, I’m starting to get tired of having my *correct* answers turned down for some reason. "Hacer ejercicio" does mean to work out, I don’t make up things. Sorry to say but Graham doesn’t know what he’s saying when he says this isn’t correct Spanish and neither do the others who say the same. Do have it however you wish, anyway, who cares anymore? Double sheesh!

It means someone who doesn’t know Spanish has put the phrase "needs to do a little exercise" into an online translator and has come up with this meaningless phrase which would probably mean little to a native Spanish speaker.
It should be "Necesita hacer un poco de ejercicio"

Literal English translation: As you’ve written the phrase in Spanish you omitted the "de" between ‘poco’ and ‘ejercicio.’ It is still correct in Spanish but the literal tranlation becomes: "Needs to do little exercise."
What it means is: "It is necessary to workout even if it is just once in a while."
